In New York, death records are available from the state and from the
local registrars of the municipality where the death occurred. Death
records for New York outside of New York City are available at the New
York State Department of Health.
Individuals who are permitted to request a copy of a death certificate:
� Legal representative of the estate of the individual named on the
death certificate.
� Immediate family members including spouse, son, daughter, mother, or
father.
� Individuals who have a legal claim or lawful right.

� Additional information may be required from extended family members

� Items for which you do not know the correct response should be filled

� Identification for the requestor must be provided with the request.
One of the following items must be provided: driver�s license,
non-driver photo ID card, passport, or any other government issued photo
ID. If you do not have one identification form from those, then you must
submit two of the following: a utility bill, telephone bill, or a letter
from any government agency that is dated within the previous six months.
Each of the bills or letter must clearly indicate the applicant�s name
and address.
Fees associated with acquiring a copy of an official death certificate
from the New York State Department of Health:
� A $30.00 fee per copy for civilians. This fee covers the initial
search and either one copy or a statement indicating that no record was
found.
� If priority handling is requested, an additional $15.00 will be
charged.
� Mail orders must be paid by check or money order make payable to the
New York State Department of Health.
� Online, telephone, and fax orders must be paid by credit card. This
type of order is considered priority handling and will be assessed the
additional $15.00, as well as an additional $11.95 for the processing of
a credit card payment.
� For FedEx overnight delivery, an additional $13.00 will be charged.

How to Apply for Certified Copied of Death Certificates Online
To submit a request for a death certificate in New York online, access

In addition to the regular fee, a convenience fee of $11.95 will be

required to receive a certified copy of the death certificate through an
express carrier. An additional charge is assessed for the use of an
express carrier.
