Death Records in Kentucky
The Office of Vital Statistics maintains Kentucky�s death record for
deaths that occurred in Kentucky from 1911 to the present.
You can obtain a certified copy of a death certificate (if you are
eligible) by mail, in person, over the phone or by using the internet.
Death Certificate Fees
Each certified copy of a death record costs $6 apiece. If no death
record is found, the initial $6 fee is kept by the Office as a search
fee. Payment should be made in the form of a check or money order made
out to �Kentucky State Treasurer�. You may use a credit card for phone
or internet orders, however additional fees apply when you use these
methods.
How to order a Kentucky death certificate by mail:

Fill out the request completely and include the following information:
� Full name of the deceased
� Death date
� Kentucky county of death
� Attending physician�s name
� Funeral director
� Name and address where the death certificate shall be mailed
� Your daytime phone number (8:00-4:30 Eastern Time).
� Number of copies you are ordering
Attach the appropriate fees in check or money order form to cover the
cost of each certificate ($6 each) and mail to:

To request a Kentucky death certificate by telephone:
Call (877) 817-7362. Make sure you have the following information
available:
� Your credit card information
� Full name of the deceased
� Death date
� Kentucky county of death
� Attending physician�s name
� Funeral director
� Name and address where the death certificate shall be mailed. Make
sure to have a physical address for FedEx deliveries, they will not
deliver to a P.O. Box.
� Your daytime phone number (8:00-4:30 Eastern Time)
� Number of copies of the death certificate that you are ordering
To request a copy of a Kentucky death certificate by fax:
Fax your death certificate request to (877) 435-5584 along with:
� Credit card information
� Full name of the deceased
� Death date
� Kentucky county of death
� Attending physician�s name

� Number of copies that you are ordering
� A physical mailing address where you will receive the death
certificates.
Additional fees, including FedEx fees, apply when you order by fax.
To request a copy of a Kentucky death certificate over the internet:

To request a copy of a Kentucky death certificate in person:

Their office hours are between 8:00 AM to 3:30 PM Monday through Friday
(closed on holidays). You will need to fill out an application for the
death certificates and pay all applicable fees.
Autopsy Reports
You can also obtain a Kentucky autopsy report by sending in a written
request to the county coroner where the death occurred. In order to
obtain the name and address of the county coroner, contact the local
county government office or the funeral home or call (502) 564-4545 for
assistance.
