How to Find Idaho Death Records
Death Records in Idaho
Death records for deaths that occurred between July 1911 to the present
are maintained by the Idaho Bureau of Health Policy and Vital
Statistics. In addition, some Idaho counties have death records dating
before July 1911 but the county files only contain records of deaths
that occurred in that particular county.
Death and stillbirth records in Idaho are considered confidential
records for the first 50 years after the death. Because of this, the
person who is requesting a certified copy of a death certificate must
present a photocopy of an acceptable form of identification and may have
to prove their relationship to the deceased. Acceptable forms of ID
include:
� Drivers license with a photo and signature
� State ID card with a photo and signature
� Passport with a photo and signature
� Military ID with a photo and signature
� Tribal ID with a photo and signature
If these forms of ID are not available, other forms of ID are acceptable
including a Social Security Card or library card. However, in this case,
two forms of ID must be presented with one of them having a signature.
To obtain an Idaho certified copy of a death certificate, you can either
mail your request or go through VitalChek. VitalChek adds on additional

Information that must accompany all requests for a death certificate
includes:
� Type of certificate, in this case it�s a death certificate
� Full name on the death certificate.
� Date of death
� City of death.
� Your relation to the person whose death certificate you are ordering.
Proof of your relationship could be required. Immediate family members,
legal guardians or their legal representatives are the only people who
may order death certificates. Step family members, in-laws,
great-grandparents are not considered immediate family members by Idaho
statutes and are excluded.
� Reason for needing the death certificate.
� Number of copies of death certificates you want.
� Your return address.
� A photocopy of your acceptable identification including both sides of
the ID(s).
� Your signature
If faxing your death certificate request to VitalChek, include the

Fax your death certificate application or letter, legible photocopy of
your accepted ID and your credit card information to 1-866-559-9629. You
will incur additional service fees from VitalChek.
Apply in Online:
Visit www.VitalChek.com and follow the instructions for ordering an

Death certificates each cost $13 and each additional copy is also $13.
