How to Find Colorado Death Records

Death Records in Colorado

The Colorado Department of Public Health and Environment - Vital Records Section maintains death records for persons who died in Colorado between 1900 to the present. Access to death records is limited to the spouse, parents, grandparents, step-parents, siblings, adult children and adult grandchildren of the deceased as well as legal representatives of any of the above or to others who can demonstrate a direct, tangible interest in obtaining the death record.

In order to establish identity, eligibility and to prevent fraud, all requests for death records must be accompanied by a copy of the requester�s identification. Among some of the accepted forms of ID are:

� U.S. or Foreign Passport
� Temporary Resident Card
� Tribal ID Card
� U.S. Military ID card

How to Request a Death Certificate

In Person
You can request a death certificate in person, by mail, by fax or online.

You must supply proof of identity, pay all fees and fill out an application for the death certificate which can be downloaded here:

Applying in person generally gets you same day service.

By Mail
To apply by mail, download and fill out the death certificate application from the link provided above, send payment in the form or a check or money order made payable to Vital Records (or provide credit card information on the application), and attach a photocopy of your acceptable photo ID card. If you pay fees by credit card, you will incur an additional $6 convenience fee.

By Fax
To apply for a death certificate by fax, download and fill out the application from the link above, fill out your credit card information on the application (this will add on an additional $6 convenience fee). Fax the death certificate application, along with proof of identity, to 1-800-423-1108 or if outside the continental US, fax to 1-303-691-9307.

Online
To apply for a death certificate online you can visit either www.VitalChek.com or www.USCerts.com. Both of these private companies allow you to order your death certificates online, they accept major credit cards, they allow secure ordering and they offer express shipping. Using these services will incur additional fees.

Fees

The first certified copy of a death certificate costs $17. If no record is found the $17 charge is applied as a search fee. Each additional copy ordered at the same time costs $10 per copy.

If the year of death is unknown, $5 is charged for each additional index searched. The indexes are broken down as follows:
1900-1925
1926-1950
1951-1975
1975 to present

A $6 credit card convenience fee is added to any mailed or faxed credit card requests. Credit card charges made in the office are not charged this convenience fee.

FedEx is $12.75 and Express Mail is $14.40. Regular mail is free.

Amending a Death Certificate

Oftentimes, a death certificate may contain incorrect information and an amendment may be needed. Because of the complex laws governing changing a death certificate, it�s best to talk to a representative from the Department of Health and Environment before proceeding. You can call Death Records at 1-303-692-2188 for advice.
