Birth Records in Kentucky

The Office of Vital Statistics maintains Kentucky�s birth record for births that occurred in Kentucky from 1911 to the present. A small amount of records for births that occurred in Kentucky before 1911 are maintained here as well, though only if the person had requested a �delayed birth certificate� at some point during his or her lifetime and provided the proper documentation to establish their birth.

You can obtain a certified copy of a birth certificate (providing you are eligible) by mail, in person, using the internet, calling on telephone.

Fees

Each certified copy costs $10 apiece. If no record is found, the initial $10 fee is kept as a search fee. Payment should be in a check or money order made out to �Kentucky State Treasurer�. For phone or internet orders, you can use a credit card however additional fees apply using these methods.

How to order a Kentucky birth certificate by mail:

Fill out the application completely including the following information:

� Full name of the person on the certificate (their name at birth)
� Birth date
� Kentucky county of birth
� Mother's maiden name
� Father's name
� Name and address where the birth certificate shall be mailed
� Your daytime phone number (8:00-4:30 Eastern Time).
� Number of copies you are ordering

Attach the appropriate fees in check or money order form to cover the cost of each certificate ($10 each) and mail to:
