Birth Records in Kansas

The Kansas Department of Health and Environment�s Office of Vital Statistics maintains birth records for births that occurred since July 1, 1991. In addition, some �delayed certificates of birth� are maintained for births that occurred before that time period if these individuals were alive after 1940 and applied for a delayed birth certificate.

If you are an authorized individual, you may request a certified copy of a birth certificate or a delayed birth certificate from the Office of Vital Statistics in six different ways:

US Mail, Priority Mail, telephone, fax, internet, and walk-in

To be eligible you must either be named on the record or an immediate family member. Additionally if you can provide legal proof that the birth record is necessary for determining personal or property rights you may request a copy. If you are a legal guardian, please provide copy of guardianship papers.

You will also need to provide proper ID to establish your identity. Currently accepted forms include a current government-issued photo ID, a passport or visa, or a military ID. If you can�t provide these, alternative paperwork can be used to establish your identity. Two of the following can be provided:
� Social security number
� Bank statement showing current address
� Car registration or title showing current address
� Utility bill showing current address
� Pay stub showing your name, social security number, and the name and address of the employer

Fees:

Each record costs $12 for the first copy and $7 for each additional copy. If no record is found, the $12 is kept as a search fee. Payment can be made by check or money order made payable to �Kansas Vital Statistics�. Credit cards are accepted by phone, fax, internet or in person.

To request a birth record by mail:

All requests for birth records must be in writing. You can download an application here:
www.kdheks.gov/vital/  or send a letter with the following details:
� Let them know you are requesting a birth certificate
� Reason for request
� Type of certificate: Certified paper copy or wallet-sized card
� Number of copies or cards you are requesting
� Full name (as listed on the birth certificate)
� Date of birth (month, day, year)
� Sex
� Place of birth
� Mother's maiden name
� Father's full name
� Your relationship to the person who is named on the birth certificate
� Your daytime phone number
� Your mailing address
� Don�t forget to sign the request
� Include a legible photocopy of a government issued ID or two of the alternate documents listed above.
� Enclose appropriate fees
� Enclose business sized self-addressed stamped envelopes for each request of different records

This method takes about five to ten business days (depending on the current volume of requests) after the Office of Vital Statistics receives the request.

To request a birth certificate using Priority Mail:

Follow the instructions for requesting a certificate by mail except send your application and fees using any type of expedited delivery option of your choice and enclose a pre-paid, self-addressed priority mail envelope with your request. DO NOT send priority service account numbers or extra payments for the Office of Vital Statistics to use.

This method takes about three business days after the Office of Vital Statistics receives the request.

To request a birth certificate by phone:

Call (785) 296-3253 between 8:00AM and 4:00PM Central time Monday through Friday, excluding holidays. You will be able to pay with credit cards using the phone service however additional service fees of $9 apply. In addition, if you choose FedEx delivery, the FedEx charges will be charged to your credit card. Phone requests are handled by VitalChek. You will need the following information:

� Valid MasterCard, Visa, American Express, or Discover card information
� Your Social Security Number or Drivers License number
� Type of record you are requesting and reason for the request
� Type of certificate: Certified paper copy or wallet-sized card
� Number of copies or cards you are requesting
� Full name (as listed on the birth certificate)
� Date of birth (month, day, year)
� Sex
� Place of birth
� Mother's maiden name
� Father's full name
� Your relationship to the person who is named on the birth certificate
� Your daytime phone number
� Your mailing address and where you want the copies sent to (FedEx will not deliver to a P.O. box).
This method takes about three business days after the Office of Vital Statistics receives the request.

To request a birth certificate by fax:

You can fax your written request to (785)357-4332 anytime. VitalChek also handles faxed requests and additional fees of $9 apply. You can use this link to print a form to fax:

Or write out your request, providing the same information as required for phone orders. This method takes about three business days after the Office of Vital Statistics receives the request.

To request a Kansas birth certificate over the internet:

Go to www.vitalchek.com  and click on the state of Kansas. Fill out the required information and pay using your credit card. This method incurs the additional $9 fee along with FedEx fees. This method takes about three business days after the Office of Vital Statistics receives the request.
