Who May Apply for a Birth Record:
 
Illinois birth records are confidential records and you must be eligible to request a copy. In order to be eligible, you present a written request, must be over age 18 and be the person listed on the record or the parent or legal representative of the person listed on the record. The Federal Government, Department of State or a Municipal Corporation may present a written request and court orders are also able to request a copy. You must state your relationship on your application along with the reason why you are requesting the record.
 
How to Apply for a Birth Record:
 
All requests must be in writing. For your convenience you may download an Application for Search of Birth Record Files here:

You�ll need to include as much information as possible including the registrant�s name, date of birth, city or town of birth, county of birth, birth number if known, gender, father�s full name and mother�s full name including her maiden name.
 
You can apply in person, by mail, by fax or online.

You will need to present a picture ID proving your identity and state your relationship to the person listed on the birth record. If you are eligible, the certified copies will be mailed within two business days.

To apply online, you will use the www.VitalChek.com  web service. This vital records service allows you to order an Illinois birth record online and pay with a credit card. You will incur additional fees to use this service such as an $8.50 handling fee and a $14.50 UPS next day delivery fee. These orders typically take three to four business days to process and eligibility requirements must be met before the records are released.
 
An adult must be present for accepting and signing for the UPS delivery. Post office boxes are not acceptable return addresses for using this service.

The fax service is a service of Vital Chek and will incur additional fees such as the $8.50 handling fee and a $14.50 UPS next day delivery fee.
 
Faxed orders typically take three to four business days to process. Eligibility requirements must be met before the birth records are released. In addition, an adult must be present to sign for the UPS delivery. The mailing address can not be a Post Office box when using this service.
 
Fees:
 
Illinois offers two different types of certified copies to choose from, either a certified photo copy of the original or a certified computer generated abstract of the original record. Fees for the photocopied version cost $15 while the computer generated version costs $10. In addition, the computer abstract comes in two sizes, wallet and 6x8, and does not list the parents� names. Additional copies ordered at the same time cost $2 each.
 
All versions of the certified copy of an Illinois birth certificate are certified by the State of Illinois and are considered acceptable for legal purposes.
 
Commemorative Birth Certificates:
 
A commemorative, legal birth certificate is available for $40 with proceeds going 50% to the Illinois Child Abuse Prevention Fund and 50% going to Illinois Coalition Against Domestic Violence. This beautiful certificate measures nine inches wide by twenty inches long and features prairie-styled stained glass motif of Frank Lloyd Wright along with the state flower, the violet, and the leaf of the state tree, the white oak, woven into the handcrafted border.
