- Birth records for births that occurred from July 1911 to the present are maintained by the Idaho Bureau of Health Policy and Vital Statistics. In addition, some Idaho counties have birth records dating further back but the county files only contain records of births that occurred in that county.
Birth certificates in Idaho are confidential records for the first 100 years after the birth. Because of this, the person who is requesting a certified copy of a birth certificate must present a photocopy of an acceptable form of identification and may have to prove their relationship to the registrant. Acceptable forms of ID include:
- Drivers license with photo and signature
- State ID card with photo and signature
- Passport with photo and signature
- Military ID with photo and signature
- Tribal ID with photo and signature
If the above forms of ID are not available, other forms of ID are acceptable such as a Social Security Card or school ID card. However two forms of ID must be presented and one of them must have a signature.
To obtain a certified copy of a birth certificate in Idaho, you can either mail your request or go through a service called VitalChek. Mailing your request is the less expensive way to order these records because VitalChek tacks on additional service fees as well as mail fees.
Information that must accompany all requests for a birth certificate includes:
- Type of certificate, in this case its a birth certificate
- Full name on the birth certificate.
- Date of birth
- City of birth.
- Father's full name
- Mother's full maiden name.
- Your relation to the person whose birth certificate you are ordering. Proof of your relationship may be required. (Only immediate family members, legal guardians or their legal representatives may order birth certificates.)
- Reason for needing the birth certificate.
- Number of copies of birth certificates you want.
- Your return address.
- A photocopy of your acceptable identification, showing both sides of ID(s).
- Your signature
If you are faxing your request to VitalChek include the following additional information:
- Delivery method - Regular mail or Express Service. Your return address must be a street address for Express Service delivery.
- Credit card number and expiration date - if you are not the person named on the credit card, a signed authorization from the credit card holder is also required.
- Daytime phone number
By Mail:
Download an application form here:
http://www.healthandwelfare.idaho.gov/
Or write a letter requesting the copies including all the information outlined above.
Mail the completed form or letter, a legible photocopy of identification, and a check or money order covering the appropriate fees to:
- Idaho Bureau of Health Policy and Vital Statistics
- P.O. Box 83720
- Boise, ID 83720-0036
By Fax Using VitalChek:
Fax your application or letter, photocopy of ID and credit card information to 1-866-559-9629. You will incur additional fees from VitalChek.
Online Using VitalChek:
Go to www.VitalChek.com and follow the prompts for ordering an Idaho birth certificate. Vital Chek authenticates your identity and processes payment in the form of a credit card. They will then notify the Idaho Bureau of Health Policy and Vital Statistics who will issue the birth records.
Fees:
Certificates cost $13 each. Additional copies are also $13 apiece. You will pay an additional $10.50 special handling fee for all online or faxed orders using VitalChek. If you wish to have Express Service, add $18 and be sure to specifically request Express Service on your application as your delivery method choice.
So long as you are an authorized person requesting the copy of the Idaho birth certificate, you have filled out the application completely, provided a legible copy of your acceptable photo ID and have enclosed the appropriate fees, your request should be completed within 5-7 business days of receipt by mail. Online and faxed orders that have Express Service noted and paid for are processed in two business days as long as they are received before 4:00 PM Mountain Time.Interested in obtaining your birth certificate?
