In order to establish identity and to prevent fraud, all requests for birth records must be accompanied by a copy of the requester�s identification. Among some of the accepted forms of ID are:

How to Request a Birth Certificate

In Person

You can request a birth certificate in person, by mail, by fax or online.

To request a birth certificate in person, you can visit their office between 8:30 a.m. to 4:45 p.m., Monday--Friday:

By Fax

To apply by fax, download and fill out the application from the link above, fill out the credit card information on the application (this will cost an additional $6 convenience charge). Online

To apply for a birth certificate online you can visit either www.VitalChek.com or www.USCerts.com. Both of these private companies allow you to order your birth certificates online and accept major credit cards, allow secure ordering and offer express shipping. Using these services will cause you to incur additional fees.

Fees

The first certified copy costs $17. If no record is found the $17 charge is applied as a search fee. Each additional copy ordered at the same time costs $10 per copy. A $6 credit card convenience fee is added to any mailed or faxed credit card orders. Credit card charges made in the office are not charged this fee.

Amending a Birth Certificate

Oftentimes, a birth certificate may contain incorrect information and an amendment may be needed. Because the laws governing changing a birth certificate are complex and court orders may be required, it�s best to talk to a representative from the Department of Health and Environment before proceeding. You can call Birth Records at 1-303-692-2226 for advice.
