Alabama maintains records of birth at the Alabama Center for Health Statistics and has birth records available for persons born in Alabama from 1908 to present.
In Alabama, birth certificates are considered confidential records and are restricted by Alabama law for 125 years after the date of birth. Only the following people are authorized to obtain copies of Alabama birth records: the person who is listed on the certificate, the parents or legal guardian of the person, a spouse, sibling or child of the person on the certificate, or a legal representative of an authorized individual.
How to Obtain a Copy of an Alabama Birth Certificate
If you are an authorized individual wishing to request a certified copy of a birth certificate, fill out the application form found at: http://ph.state.al.us/chs/VitalRecords/Universal/dl.pdf
Include a check made payable to �Vital Records� for $12 for the first copy of the birth certificate and $4 for each additional copy. You will receive either a certified copy of the birth certificate or a �Certificate of Failure to Find� if they were unable to locate the document. Either way, your fees are non-refundable.
In order to help the Center for Health Statistics to locate the record, provide as much information as possible including:
- Full name of person at birth
- Date of birth
- Sex
- City (or county) of birth
- Hospital of birth � or "home" for home deliveries
- Full maiden name of mother and full name of father
- Your name, signature and relationship to the person whose certificate you are requesting
- Address where they shall mail the certificate
- Your daytime phone number

You may request copies of birth certificates by going to any county health department in person. Many of these offices can fill your request while you wait.

After an adoption, legitimation or paternity determination is made, Alabama law directs the State Registrar must issue a new birth certificate and seal the original. An adult adoptee or other person whose original birth certificate was placed in a "sealed file" may obtain a non-certified copy of that record and any other documents in the sealed file. Birth parents are able to state their preferences about future contact and can provide medical information. Amendments to Birth Certificates
Amendments can be made to a birth certificate if information needs to be corrected or added. You must fill out the appropriate form and send in the proper fees of $15 to:

Paternity amendments can be made under Alabama law under the following conditions:
- Court ordered legitimation
- Legitimation through marriage
- Affidavit of Paternity
- Uniform Parentage Act
Each of the conditions of paternity has specific legal requirements that must be met. You must fill out a form and send $20 to:
