Death Records in Alaska
The Department of Health and Social Services of the
State of Alaska maintains records of deaths in Alaska
Death records in Alaska may be requested only by
the spouse, parent, child, or sibling of the person listed on the death
certificate. Others may request death certificates however a letter from
the office or agency who needs the death certificate must accompany the
application
To prevent identity fraud and to verify that an
applicant is eligible to receive a copy of the death certificate, a copy
of a picture ID along with a signature on the copy must accompany the
request.
How to request a copy of a death certificate:
First, download the form here:

Next, make a copy of your picture ID. Enlarge it
and lighten it if possible so that it is clear and easy to read. Once
you have a satisfactory copy, sign the copy.
Fill out the form and attach the appropriate fees.
You can pay by check if you have checks with your name and address
pre-printed on them. If not, get a money order or pay by credit card.
You will incur an additional $11 fee for paying by credit card. This
does not expedite your order.
Mail the form, your signed copy of your picture ID
and the appropriate fees to:

If you are faxing your request, fax your form,
signed copy of your picture ID and credit card information to:
907-465-3618.
After faxing your request, please call the Alaska
Bureau of Vital Statistics to make sure they received the fax. The
number is 907-465-3391 and the hours are from 8:00 AM to 4:00 PM Alaska
Time, Monday through Friday.
Fees
Each certified copy of an Alaskan death certificate
costs $20. This fee is non-refundable however, if the certificate cannot
be located, a 3 year search along with a statement of search will be
issued in its place. Extended searches cost $1 per year.
Additional fees come into play for death
certificates requiring authentication for a foreign country. The charge
is an additional $12 for the first record which includes a $2 fee for
the Lt. Governor�s office. Additional copies of the same record cost $2.
You must note the country that the record is being sent to on your
request.
Processing Time
Mailed applications usually take about 3-5 business
days from the time they arrive at the Bureau of Vital Statistics. Faxed
requests paid for with a credit card are usually processed within 3
business days. Allow additional time for the mail service to deliver
your certificate.
Please note that the $11 credit card fee is not
intended to expedite service.
Electronic Requests
www.VitalChek.com is an option for those who prefer
to request the death certificate electronically. When you use this
service you will incur fees in addition to pay the $20 fee that Alaska
requires along with an expedite fee, shipping fee and VitalChek�s
processing fee. Using VitalChek is easy but the processing times are set
by the agency who has the records. VitalChek has no control over the
agency�s workload or the time the particular agency takes to process and
ship the certificate.
