How to Find Alabama Death Records

Death Records in Alabama

The Alabama Center for Health Statistics maintains death records for people who have died in Alabama since 1908. Alabama law considers death records confidential and limits access to them for 25 years after the date of death. Death records over 25 years old can be obtained by anyone who is able to pay the fee.

Authorized persons may obtain a restricted death certificate if they pay the proper fee and are any of the following individuals:
� A parent of the person listed on the death certificate.
� A spouse of the person listed on the death certificate.
� A child of the person listed on the death certificate.
� A grandchild of the person listed on the death certificate.
� A sibling of the person listed on the death certificate.
� A legal representative of the estate or the family of the person listed on the death certificate.
� Informant listed on the death certificate as providing information.

The non-refundable fees for death certificates cost $12 with each additional copy costing $4 each. You will receive a certified copy of the death certificate or a certificate of �Failure to Find� indicating the death certificate was not found. Do not send cash, instead send a check or money order made out to Vital Records for the proper amount.

How to Obtain a Death Certificate

Fill out the application completely. Be sure to include the following information to help the Center for Health Statistics find the records you need:

� Full legal name of person who died in Alabama
� Date of birth or age at death
� Date of death
� Sex
� Social Security number
� City (or county) of death
� Race
� Name of spouse
� Names of parents
� Your name, daytime phone number and signature
� Your relationship to the deceased whose certificate you are requesting
� Address where the certificate is to be mailed

You may request copies of death certificates by going to any county health department in person. Many of these offices can even fill your request while you wait.

How to Amend a Death Certificate

To amend a death certificate in the case of an error or additional information, you must follow several specific steps. Because the death certificate is a legal document, specific steps must be followed. Medical information changes can only be made by the medical certifier who signed the death certificate.

Non-medical information may be amended by the spouse, parent, informant, funeral home (or other person with the appropriate legal authority) of the decedent who is listed on the certificate. The original death certificate remains unchanged however an affidavit with the corrections to the certificate will be attached.
