How to Find Kentucky Death Records

Death Records in Kentucky

The Office of Vital Statistics maintains Kentucky’s death record for deaths that occurred in Kentucky from 1911 to the present.

You can obtain a certified copy of a death certificate (if you are eligible) by mail, in person, over the phone or by using the internet.

Death Certificate Fees

Each certified copy of a death record costs $6 apiece. If no death record is found, the initial $6 fee is kept by the Office as a search fee. Payment should be made in the form of a check or money order made out to “Kentucky State Treasurer”. You may use a credit card for phone or internet orders, however additional fees apply when you use these methods.
