How to request a Georgia birth certificate:

All requests must be in writing and contain the following information:

• Full name of the person shown on the birth certificate (maiden name at birth if female)
• Date of Birth (month, day and year)
• Place of Birth (city and county),
• Current age
• Sex
• Race (optional)
• Full name of the mother (include mother's maiden last name)
• Full name of the father
• Requester�s relationship to the person listed on the birth certificate requested
• The number of certificates requested

You can also make a request in person by visiting the State Vital Records Office or the County Vital Records Office in the county where the person was born.

Online, phone and fax:

You can request a Georgia birth certificate through www.VitalChek.com, a web service designed to assist you in obtaining vital records such as birth certificates. Using this service does add to your total cost. You will incur additional fees such as service fees and shipping fees. Vital Chek accepts all major credit cards.

Fees

Fees for birth records are non-refundable. Your $10 search fee will include a certified copy of the birth certificate if it is found. If it is not found, the State of Georgia retains the $10 fee for the search. Additional copies cost $5 each if ordered at the same time. If you are unsure of the year, you will pay $10 for each three year multi-year search.

Special filing fees apply when requesting a delayed certificate of birth or an amendment to a birth certificate. An additional $10 special filing fee applies when you request a delayed certificate. Amendments for adoption, paternity, legitimation or a court order also have this additional $10 fee. Amendments during the first year of birth however are not charged a special filing fee.

For birth certificates requested by mail or in person, acceptable forms of payment are certified checks or money orders made out to Vital Records.

Credit cards are only accepted when you go through the Vital Chek Network.
